Asks about your general health, any medicines you take, allergies, and anything a previous dentist has treated. Bring the pharmacy printout if your list is long.
Consent to treatment
Signed at the appointment, once the plan and the costs are in front of you.
Records release
If you would like X-rays and notes requested from a previous dentist, this authorises them to send them here.
