What we collect
Your name, contact details, health card number, insurance details, health history and the clinical records created while treating you.
Why
To provide dental care, to bill your insurer where you have asked us to, and to meet the record-keeping obligations set by the Royal College of Dental Surgeons of Ontario.
Who sees it
The dentist and staff involved in your care. Your insurer sees only what a claim requires. We do not sell information to anyone.
How long we keep it
Dental records are retained for the period required by Ontario regulation, then destroyed securely.
Your rights
You may ask to see your record, ask for a correction, or ask that it be sent to another dentist. Write to info@allcaredental.ca.
